This role also involves planning and coordinating medical visits, maintaining electronic patient records using CareCoach, and overseeing the care process, including leading case discussions. Additionally, the position includes acting as a liaison for specific topics, coordinating daily operations, and managing a team. To excel in this role, candidates must have completed training as a Dipl. Pflegefachfrau/Pflegefachmann HF and a Berufsbildnerkurs, as well as a SVEB 1 certification. Experience in long-term care is preferred, and strong communication skills in German are essential.
